	Background information on applicant (description of your disability, education, employment, etc.)
	Outline of research project (subject, significance, description of how research will be undertaken)
	Letter of support for this application

For your application to be considered, all of the above information must be sent to the Office of the Clerk of the Committee on Human Resources, Social Development and the Status of Persons with Disabilities and must be postmarked on or before May 29, 2007.
